Qualifications

• Previous experience as a Resident Services Coordinator in senior living.
• Experience in Assisted Living or Memory Care settings.
• Knowledge of care workflows and team communication.
• Experience with electronic health records (EHR).
• Strong organization and attention to detail.
• Clear, compassionate communication with residents and families.

Responsibilities

• Coordinate staffing, scheduling, and daily coverage.
• Serve as point of contact for residents and representatives regarding care coordination.
• Communicate changes in condition and care plans to nurses and staff.
• Monitor charting and care notes for accuracy.
• Coordinate resident appointments with physicians and outside providers.
• Support service planning and care transitions.
• Provide day-to-day leadership to caregivers and medication technicians.
• Assist with documentation and regulatory compliance.
