Job summary

The Springs Living is looking for a Resident Services Coordinator (RSC) to join our Health Services team. This role is crucial for coordinating resident care and working closely with the community nurse and caregiving team.

Ideal candidates will have direct-care experience, particularly as a Medication Technician or Resident Services Coordinator. The RSC will manage care plans, communicate changes in resident conditions, and ensure high-quality support for all residents.

Qualifications

• Previous experience as a Medication Technician or Resident Services Coordinator in senior living.
• Hands-on experience supporting resident care in Assisted Living or a similar setting.
• Strong understanding of care workflows, documentation, and team communication.
• Ability to work comfortably with electronic health records and care management systems.
• Excellent organizational skills and attention to detail.
• Clear, compassionate communication skills with residents, caregivers, and leadership.

Responsibilities

• Serve as a primary point of contact for residents and their authorized representatives regarding care coordination.
• Communicate changes in condition and care plans to nurses, med techs, and caregiving staff.
• Monitor and follow up on charting, care notes, and communication logs for accuracy and completion.
• Coordinate resident appointments with physicians and outside healthcare providers.
• Support service planning, care transitions, and ongoing care coordination.
• Assist with staffing coordination, scheduling adjustments, and coverage as needed.
• Provide day-to-day leadership support to caregivers and medication technicians.
• Ensure documentation and processes meet regulatory and company standards.
