Job summary

Twin Cities Physicians is seeking a Physician (MD/DO) for senior living community-based primary care across Minnesota. The Senior Living Rounding role involves on-site care in assisted living, memory care, skilled nursing, and long‑term care settings, delivering proactive, relationship-based medicine.

You will lead clinical decisions, mentor team members, and influence care across a broad senior population, with a strong emphasis on reducing avoidable hospitalizations and enhancing patient

Qualifications

• Active Minnesota Physician (MD or DO) license in good standing.
• Experience in geriatrics, family medicine, internal medicine, post‑acute care, long‑term care, or related specialties preferred.
• Strong clinical judgment and confidence managing medically complex patient populations.
• Interest in collaborative, community‑based medicine.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide on-site rounding and primary care services throughout assisted living, memory care, skilled nursing, and long‑term care communities.
• Manage complex medical conditions, chronic disease management, preventative care, and transitional care needs.
• Develop treatment plans and guide clinical decision‑making across diverse patient populations.
• Collaborate with advanced practice providers and interdisciplinary teams to ensure continuity of care.
• Support quality initiatives aimed at reducing emergency room utilization and hospital readmissions.
• Communicate effectively with residents, families, caregivers, and community leadership.
• Complete documentation, medication management, and clinical oversight responsibilities.
