Qualifications

• Valid driver’s license and auto insurance.

Responsibilities

• Conduct regular rounding visits in assisted living and memory care environments.
• Perform comprehensive medical assessments and manage complex conditions.
• Develop personalized care plans in collaboration with patients and families.
• Work closely with NPs, PAs, and care teams to ensure seamless coordination.
• Educate residents and their loved ones about care options and goals.
• Proactively manage chronic conditions to reduce hospital admissions.
