Job summary

Twin Cities Physicians is seeking a Physician Assistant (PA-C) to serve as a Mobile Rounding Provider within senior living communities across Minnesota. This role focuses on continuity of care, chronic disease management, and proactive interventions to improve patient outcomes.

You will be integrated into a provider-led model, delivering on-site primary care and geriatric services, with opportunities to develop expertise in geriatrics and post-acute care while enjoying a mobile practice

Qualifications

• Active Minnesota PA-C license in good standing.
• Experience in geriatrics, family medicine, internal medicine, hospital medicine, home health, or post-acute care preferred.
• Strong clinical judgment and communication skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide on-site rounding care in assisted living, memory care, and long-term care communities.
• Perform assessments, chronic disease management, preventative care, and follow-up visits.
• Develop and adjust patient-centered care plans.
• Collaborate with physicians, nurses, therapists, caregivers, and interdisciplinary teams.
• Communicate effectively with families, caregivers, and facility staff.
• Help reduce avoidable hospitalizations through proactive clinical management.
• Complete documentation, medication management, and lab coordination.
