Twin Cities Physicians is a provider-led medical group delivering on-site primary care, geriatric medicine, and chronic disease management across 300+ senior living communities throughout Minnesota. Our physicians care for residents in assisted living, memory care, skilled nursing, and long-term care settings, bringing high-level medical care directly to patients while improving access, continuity, and outcomes.
Our model is built around proactive intervention, relationship-based medicine, and reducing avoidable hospitalizations. Rather than reacting to healthcare crises, our providers help prevent them.
Most physicians enter medicine to make a lasting difference.
Over time, many find themselves spending more time navigating systems than practicing medicine.
This opportunity offers something different.
As a Senior Living Rounding Physician, you'll become a trusted medical leader across the communities you serve. You'll care for complex patients, guide treatment decisions, mentor and collaborate with advanced practice providers, and help shape the overall quality of care delivered throughout your communities.
This is more than a patient panel.
It's an opportunity to influence outcomes across an entire population of seniors while practicing medicine in a highly relationship-driven environment.
We proudly encourage active duty, reserve, National Guard, and veterans to apply.
Healthcare is changing, and the future of senior care will be shaped by physicians who are willing to move beyond traditional models and build something better.
If you're looking for an opportunity where your experience, leadership, and clinical expertise can influence not only individual patients but entire communities, Twin Cities Physicians offers a platform to make that impact.
