Job summary

Sagora Senior Living is seeking a Resident Services Coordinator (LPN/LVN) to lead staffing and care coordination in one of our senior living communities. The role involves coordinating staffing needs, developing care plans, working with physicians and discharge planners, and communicating with family members to ensure high-quality care for residents.

Qualifications

• Must be a graduate of an accredited school of nursing (LPN/LVN).
• 1+ year of management experience in Assisted Living or Memory Care.
• Effective communication with residents, families, physicians and staff.

Responsibilities

• Assist with recruitment, hiring, training, supervision, and evaluation of caregivers to ensure quality services.
• Plan and delegate work assignments with effective scheduling.
• Assist the Resident Services Director in providing case management services to residents.
• Coordinate wellness activities and participate in resident move-in/move-out processes.
• Monitor regulatory compliance and participate in management surveys.
