Job summary

Qualifications

• Bachelor's, Master's or Doctorate in Physical Therapy from an accredited program.
• Current Physical Therapist license in the state employed.
• Current CPR certification.
• Six months of experience in a long-term care environment preferred.

Responsibilities

• Establishes and administers a treatment plan with therapy goals aligned to physician guidance.
• Assesses patient needs; plans, evaluates and modifies care to meet goals.
• Collaborates with disciplines to plan and evaluate team goals for each patient.
• Sets realistic and achievable goals for patients/residents.
• Documents and records condition and progress per regulations.
• Educates patients and families in therapy methods.
• Oversees screening to optimize patient functionality.
• Communicates status and needs to patient, family, and professionals.
• Ensures punctuality and regular attendance for shifts.
