Job summary

Reform Rehab is seeking a Physical Therapist (PT) and/or Occupational Therapist (OT) to provide mobile outpatient therapy at Pegasus Landing of Chisholm Trail in Keene, Texas. The role focuses on serving older adults in an assisted living setting with flexible weekday coverage.

Candidates must hold a current Texas PT or OT license, graduate from an accredited program, and be dependable with strong communication and documentation skills.

Qualifications

• Active Texas PT or OT license in good standing.
• Graduate of an accredited PT or OT program.
• CPR certification preferred.
• Experience with older adults or in senior living is a plus.
• Strong communication, organization, and documentation skills.

Responsibilities

• Provide skilled physical therapy and/or occupational therapy evaluations and treatments.
• Develop and implement individualized plans of care.
• Treat older adults with mobility, strength, balance, gait, and ADL limitations.
• For PTs: therapeutic exercise, gait training, balance training, functional mobility training.
• For OTs: therapeutic activities, ADL training, functional training, neuromuscular re-education.
• Complete timely documentation and ensure Medicare compliance.
• Communicate with patients, families, facility staff, and Reform Rehab team.
• Coordinate care with other therapy disciplines.
• Maintain a professional presence within the senior living community.
