Cassia Community is seeking a Physical Therapist to join Park View Care Center in Wells, MN. The role offers a competitive wage, benefits, and an opportunity to quickly build a full caseload in a rural environment.

New grads are welcome to apply. This is a full-time position with flexible start and end times, paying $45-$55 per hour depending on experience, plus a $7,500 bonus paid over 18 months.

Qualifications

• Must be a graduate of an Accredited Physical Therapy Program and have a bachelor’s degree.
• Must possess a current MN Physical Therapist License.
• Must possess knowledge of state, federal, and local Home Health care regulations.
• Must be able to perform essential job functions with/without reasonable accommodations.
• Must communicate in English, verbally and in writing.

Responsibilities

• Assess and diagnose patients, evaluate conditions, and develop personalized treatment plans.
• Develop and implement rehabilitation programs to improve functional deficits.
• Provide hands-on therapy and modalities as needed.
• Educate patients, caregivers, and staff on exercises, safety, and pain management.
• Monitor patient progress and adjust plans as necessary.
• Collaborate with doctors, nurses, and other therapists to ensure comprehensive care.

Position Type: Full-Time, benefits eligible position with a flexible start and end time

Wage Range:$45 - $55 / hour depending on experience.

Physical Therapist Responsibilities:

• Assess & Diagnose: Evaluate patients' physical conditions, movement abilities, and medical history to develop personalized treatment plans.
• Develop & Implement Treatment Plans: Create and execute rehabilitation programs to improve the functional deficits of the patient.
• Provide Hands-on Therapy: Perform manual therapy, therapeutic exercises, and use modalities such as but not limited to ultrasound and electrical stimulation as needed.
• Educate & Support Patients: Provide patient, caregiver, staff education as deemed clinically necessary such as but not limited to: pain management techniques and fall risk safety. Teach patients and families about exercises, injury prevention, and pain management techniques.
• Monitor Progress & Adjust Plans: Track patient outcomes, document progress, and modify treatment plans as needed.
• Collaborate with Healthcare Teams: Work with doctors, nurses, and other therapists to ensure comprehensive patient care.

Physical Therapist Qualifications:

• Must be a graduate from an Accredited Physical Therapy Program and have a bachelor’s degree
• Must possess a current accredited MN Physical Therapist License
• Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care.
• Must be capable of performing the essential functions of the job, with or without reasonable accommodations.
• Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required.
