You will evaluate residents, develop personalized plans to improve strength, balance, and independence, and collaborate with nursing, physicians, and families to support quality of life.

Qualifications

• Indiana PT license required.
• Strong clinical assessment and communication skills.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop individualized treatment plans based on resident needs.
• Provide high-quality physical therapy services focused on strength, balance, mobility, and functional independence.
• Collaborate with nursing, therapy, physicians, and interdisciplinary team members.
• Educate residents and families on treatment goals and home exercise programs.
• Complete timely and accurate clinical documentation.
• Promote the highest level of resident-centered care.
