Qualifications

• Must hold an active Indiana Physical Therapist license and be eligible to earn licensure.
• Ability to work within a continuing care retirement community across multiple care levels.
• Collaborative, interdisciplinary team approach to improve resident outcomes.

Responsibilities

• Provide physical therapy services within a continuing care retirement community.
• Collaborate with an interdisciplinary care team to develop and implement treatment plans.
• Support residents’ independence and functional goals across skilled nursing, rehab, and assisted living.
• Possibly available in a PRN capacity for occasional needs.
