SCHOFIELD&ASSOCIATES LLC in Muncie, IN is seeking a PRN Physical Therapist (PT) to join our therapy team at a senior living community. Help residents regain strength, improve mobility, and maximize independence while building meaningful relationships with residents and their families.

Qualifications

• Active Indiana Physical Therapist license.
• Strong clinical assessment and communication skills.
• Passion for working with the senior population.
• Positive, collaborative, patient-focused approach.
• Long-term care or senior living experience preferred, but not required.

Responsibilities

• Perform comprehensive physical therapy evaluations.
• Develop individualized treatment plans based on resident needs.
• Provide high-quality physical therapy services focused on strength, balance, mobility, and functional independence.
• Collaborate with nursing, therapy, physicians, and interdisciplinary team members.
• Educate residents and families on treatment goals and home exercise programs.
• Complete timely and accurate clinical documentation.
• Promote the highest level of resident-centered care.
