Responsibilities include delivering daily PT treatments, implementing care plans, and ensuring compliance with regulatory and company policies while coordinating with an interdisciplinary team to meet resident needs.

Qualifications

• Minimum 1 year experience working with older adults preferred.
• Current CPR certification is required.
• Current state license in physical therapy where applicable.

Responsibilities

• Provide holistic care across physical, spiritual, emotional and clinical needs.
• Deliver day-to-day physical therapy treatments in all settings within the community.
• Follow Medicare, state and federal regulations and company policies.
• Implement residents’ personal care plans focusing on preferences and routines.
• Collaborate with interdisciplinary clinical team to address residents' needs.
