Optum is seeking a part-time or PRN Occupational Therapist for a Senior Living community. The OT will provide age-appropriate services to adult and geriatric patients, including assessments, direct treatment, supervision of COTA/tech, home exercise program development, and family education.

Responsibilities include team conferences, adaptive equipment evaluation, PI participation, and documentation per facility standards.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or able to obtain within 90 days of hire.

Responsabilités

• Evaluates the patient to determine rehab potential and develops an individualized treatment plan.
• Sets initial short- and long-term goals.
• Utilizes equipment safely and maintains logs and safety records.
• Documents D/C summary on final outpatient visits.
• Provides age-appropriate OT services and supervises staff delivering treatment.
