Job summary

Discovery Village Des Plaines is seeking an experienced Licensed Practical Nurse (LPN) to provide compassionate nursing care to residents in a senior living community. You will follow individualized care plans, monitor health status, and collaborate with an interdisciplinary team to promote safety, dignity, and well-being.

Responsibilities include medication administration, care planning support, and accurate clinical documentation.

Qualifications

• Current LPN license (or RN per state regulations).
• Minimum of two years of licensed nurse experience required.
• Experience in assisted living, long-term care, or skilled nursing preferred.
• Strong clinical judgment, problem-solving and decision-making skills.
• Effective organizational, time management, and communication skills.

Responsibilities

• Provide nursing care to residents per care plans, policies, and procedures.
• Conduct ongoing assessments and monitor changes in condition.
• Administer medications and treatments as prescribed and document appropriately.
• Coordinate care with physicians, families, pharmacies, and other providers.
• Support health promotion, disease prevention, and wellness initiatives within the community.
• Ensure accurate, timely, and compliant clinical documentation.
• Assist with care planning, service plan updates, and resident evaluations.
• Collaborate with caregivers, wellness team, and leadership to ensure high-quality care.
• Respond to resident concerns, incidents, and emergencies with sound judgment.
• Support training and guidance for care staff as appropriate.
