Wind Crest by Erickson Senior Living is seeking a Clinical Liaison for our Continuing Care Retirement Community (CCRC) to coordinate data collection, transfers, and discharge planning. You will work with physicians and care teams to assess referrals and ensure smooth transitions for residents, supporting both Independent Living and Continuing Care pathways.

The role requires familiarity with acute and sub-acute care settings and delivers growth opportunities within a mission-driven organization

Qualifikationen

• CCRC, Skilled Nursing, Memory Care / Assisted Living experience highly preferred.
• Liaison experience or healthcare marketing background preferred.
• Familiarity with acute hospitals, rehabilitation hospitals and local healthcare market preferred.

Aufgaben

• Perform onsite clinical collection of data of referrals to determine transfer appropriateness.
• Perform discharge planning and care coordination for residents transferring from hospital to Continuing Care.
• Collaborate with physicians regarding anticipated discharges from facilities.
• Identify quality issues and participate in quality management activities with Administrator/Medical Director.
• Coordinate admissions data for referrals from inpatient acute care and sub-acute settings.
• Collaborate with Physicians, Continuing Care Admissions Staff, Hospital Case Managers and Discharge Planners to facilitate admissions/transfer.
