Wind Crest by Erickson Senior Living in Highlands Ranch, CO is seeking a Clinical Liaison to coordinate admissions for referrals from inpatient acute care and sub-acute settings. You will collect data and assess appropriateness for transfer to Continuing Care.

You will collaborate with physicians, discharge planners, and hospital staff to facilitate admissions and transfers, ensuring smooth care transitions and alignment with high-quality, person-centered care at our continuing care community.

Formación

• CCRC, Skilled Nursing, Memory Care / Assisted Living experience highly preferred.
• Liaison experience or healthcare marketing background preferred.
• Familiarity with acute hospitals, rehabilitation hospitals, sub-acute rehabilitation facilities, and the local healthcare market preferred.

Responsabilidades

• Perform onsite clinical collection of data of the referrals to determine appropriateness for transfer to Continuing Care.
• Perform discharge planning and care coordination for residents transferring from acute hospital to Continuing Care or returning to Independent Living at the Erickson Living community.
• Collaborate with physicians regarding anticipated discharges from facilities.
• Identify quality issues and participates in quality management activities, as needed, with Administrator and/or Medical Director.
