Job summary

Pleasant Point Senior Living in Racine, WI is seeking a Clinical Care Director to lead our CBRF & RCAC, delivering compassionate, resident‑centered care while building a strong clinical culture.

You will oversee clinical operations, resident assessments, service planning, and quality initiatives; mentor a multidisciplinary team; ensure regulatory compliance, documentation accuracy, and effective risk management.

Qualifications

• Active LPN license preferred.
• 1–3 years of leadership experience in assisted living, senior living, or healthcare.
• Strong clinical assessment, communication, and organizational skills.
• CPR certification is required.

Responsibilities

• Lead and oversee all clinical operations and resident care services.
• Conduct resident assessments, service planning, and ongoing evaluations.
• Ensure compliance with state regulations, resident rights, HIPAA, and infection control standards.
• Recruit, coach, mentor, and develop a high-performing clinical team.
• Partner with residents, families, physicians, pharmacies, and therapy providers to coordinate care.
• Monitor quality outcomes, medication management, clinical documentation, and risk reduction initiatives.
• Support occupancy efforts through resident assessments and admission evaluations.
• Foster a culture of accountability, compassion, and excellence in resident care.
