Job summary

Monarch Communities is seeking a Care Manager to join our per diem team. The role reports to Health & Wellness Director and focuses on assisting residents with daily living tasks as outlined in care plans, including personal care, meals, mobility, and social activities.

This hourly, non-exempt position offers flexible shifts and paid training. Ideal candidates are CNAs or CHHAs with experience working with elderly residents, including dementia experience.

Qualifications

• Certified Nursing Assistant or Certified Home Health Aide.
• Knowledge and experience in working with the elderly population, preferred dementia experience.

Responsibilities

• Provide resident care to residents that include bathing, dressing, toileting, and feeding, as outlined in their care plan
• Maintain the safety, comfort, dignity, and confidentiality of all residents
• Escort residents to meals and activities
• Ensures all assigned residents are clean, neatly dressed and groomed, safe, and fed during assigned shifts
• Document all services provided to your assigned residents during your shift
• Report any change of condition in a resident immediately to your supervisor
