Job summary

Cogir Senior Living in Scottsdale, AZ, seeks a Resident Care Coordinator to act as a liaison among residents, families and the care team, improving quality of life and safety. You will support the Health and Wellness Director with scheduling, staff supervision, and ensuring systems are in place for excellent assisted living services.

This role includes medication administration support, problem resolution, and coordinated care with external providers, while aligning with community regulations

Qualifications

• High School Diploma or equivalent.
• Must have a valid state medication administration license (CNA/CMA/CMT/QMAP) or meet state requirements for medication administration in assisted living.
• An LPN/LVN nursing license is a plus.
• Current First Aid and CPR license or ability to obtain.

Responsibilities

• Schedule and assign caregiving and medication technician shifts to ensure adequate coverage.
• Participate in recruitment and interview process for new care team members.
• Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
• Supervise the medication room and orders, collaborating with medication technicians and caregivers.
• Maintain accurate resident records, including care plans and medical documentation.
• Coordinate care planning with home health agencies on-site, within community policies.
• Coordinate admissions with the sales team, including supervising move-ins.
• Complete family/resident admission interviews.
• Facilitate effective communication among residents, families, and healthcare providers.
• Address residents' special needs, requests, and complaints.
• Assist with medication administration and provide direct care as needed.
• Understand state care regulations.
