Job summary

Cogir Senior Living in Scottsdale, AZ seeks a Resident Care Coordinator to liaison between residents, families, and the care team, supporting the Health and Wellness Director in scheduling, supervision, and quality service delivery.

You will help with medication administration, coordinate with external providers, and assist with admissions and care planning to ensure a safe, compassionate living environment for residents.

Qualifications

• High School Diploma or equivalent.
• Valid state medication administration license (CNA/CMA/CMT/QMAP) or meet state requirements for medication administration in assisted living.
• An LPN/LVN nursing license is a plus.
• Current First Aid and CPR license or ability to obtain.

Responsibilities

• Schedule and assign caregiving and medication technician shifts to ensure adequate coverage and compliance with staffing guidelines.
• Participate in recruitment and interview process for new care team members.
• Supervise, train, and support care team members, promoting teamwork and adherence to care standards.
• Supervise the medication room and orders, collaborating with medication technicians and caregivers.
• Maintain accurate and up-to-date resident records, including care plans and medical documentation.
• Coordinate care planning with home health agencies on-site and with the sales team for admissions.
