Job summary

Jaybird Senior Living seeks a Resident Services Coordinator to lead nursing and activity support in a patient-focused environment. You will serve as a point of contact for residents and staff, advocate for care standards, and help implement the community vision with compassion and professionalism.

The role emphasizes collaboration with clinical teams, adherence to ISP instructions, and ongoing resident engagement. Strong communication and organizational skills are essential for success.

Qualifications

• One to three years of healthcare experience or equivalent education.
• One to three years of managerial experience, preferably in healthcare.

Responsibilities

• Obtain vital signs and note deviations per ISP in the EHR system.
• Follow residents' ISP and daily task sheets.
• Review clinical notes in the EHR for completeness and accuracy.
• Communicate changes in resident status to supervisors and clinical staff.
• Maintain a safe, clean resident environment and comply with PPE.
• Assist with activities and encourage resident engagement.
• Uphold confidentiality of residents and coworkers (HIPAA).
• Provide assistance with medication management, hygiene, transfers, nutrition, dressing, ambulation, positioning, and reminders.
• Perform other duties within scope of practice.
