Job summary

Fairport Rehab & Nursing Center in Perinton, NY is seeking a compassionate Activities Aide to plan and deliver meaningful recreation for residents in a long-term care setting. You will organize calendars, lead crafts, games, music, and celebrations, and support transportation and social engagement.

Responsibilities include documenting attendance and care plans in the EMR, collaborating with the interdisciplinary team, and maintaining a warm, safe environment.

Qualifications

• Associate’s or bachelor’s degree in Recreation or related field preferred.
• Relevant education and/or experience may be considered.
• Previous experience with older adults preferred.
• Experience with Alzheimer’s disease or dementia preferred.
• Long-term care or geriatric experience preferred.
• Proficiency with Word and Excel.
• Ability to navigate an EMR system.
• Strong time-management and organizational skills.
• Strong communication and interpersonal skills.
• Dependable, creative, compassionate, flexible, team-oriented.
• Ability to lift up to 50 pounds and assist with mobility.
• Able to work evenings, weekends, and holidays as needed.

Responsibilities

• Plan and implement activities based on residents’ preferences and needs.
• Create and maintain monthly activity calendars for your unit.
• Encourage participation and assist with transportation as needed.
• Assist with crafts, games, music, religious activities, entertainment, celebrations, and events.
• Plan and coordinate facility-wide activities with the team.
• Complete attendance records and EMR documentation, progress notes, and care plans.
• Participate in interdisciplinary meetings and communicate changes to staff.
• Maintain a warm, welcoming, and safe environment for residents.
• Coordinate room setup and decoration for seasons, holidays, and events.
• Coordinate with departments and assist volunteers as needed.
• Assist with reception desk duties, mail, and other responsibilities.
