You will collaborate with the Healthcare Team to tailor plans to individual needs, encourage participation, and maintain records of attendance and progress. This role supports quality care and a compassionate, home-like environment.

Aufgaben

• Plan and implement activities that promote socialization, recreation, and therapy for patients and residents, including crafts, games, and exercise programs.
• Collaborate with the Healthcare Team to develop and update activity plans based on individual patient and resident needs.
• Engage with patients and residents to encourage participation, build relationships, and promote a sense of community.
• Maintain accurate records of activities, attendance, and progress.
• Participate in quality improvement initiatives to ensure the highest level of care and services.
• Support staff in delivering care and services, as needed.
