Legacy Community Health is seeking a dedicated manager to lead the Ryan White Case Management Team in Houston. You will oversee a multidisciplinary team delivering essential medical and non-medical services to individuals living with HIV/AIDS, ensuring trauma-informed, patient-centered care and strong community links.

You will drive quality improvement, supervise staff, ensure policy alignment, and collaborate with partners to enhance access, efficiency, and outcomes.

Qualifications

• MSW from CSWE-accredited program required.
• LMSW licensure required per Texas State Board.
• 3 years paid post-degree social work experience.
• 2 years supervisor/manager experience.
• Experience with Ryan White program or HIV services preferred.
• Leadership in program management and quality improvement desirable.
• Experience supervising across multiple locations preferred.
• Excellent communication and customer service skills.
• Strong MS Office skills and proven EHRS competency.
• Knowledge of public benefits landscape and access facilitation.
• Ability to manage multiple deadlines and handle confidential information.

Responsibilities

• Provide direct supervision and daily activity management for case management staff.
• Oversee Ryan White Case Management services ensuring compliance and quality.
• Interpret and implement program policies aligned with organizational values.
• Monitor performance metrics linked to patient outcomes and drive improvements.
• Recruit, train, and evaluate licensed and unlicensed staff.
• Promote HIV care competency, trauma-informed practices, and cultural humility.
• Support professional development and foster a collaborative environment.
• Ensure comprehensive care plans addressing diverse patient needs.
• Monitor service delivery for access, efficiency, and quality.
• Guide on complex cases, ensure eligibility and documentation adherence.
• Lead quality management including audits and data reviews.
• Maintain accurate documentation and reporting practices.
• Analyze program data for improvements and trends.
• Collaborate with teams and partners for coordinated care.
• Support billing and budget processes aligned with funding.
