Job summary

Fred Hutchinson Cancer Center in Seattle seeks a Senior Pharmacist to provide pharmacotherapeutic recommendations, direct patient care, dispensing and education within the specialty pharmacy team.

This full-time role operates Monday–Friday at South Lake Union with remote work options. WA Pharmacist License and 2 years of pharmacy practice are required; PharmD preferred. Relocation assistance may be available.

Qualifications

• Graduation from an accredited School of Pharmacy.
• WA Pharmacist License.
• Two years of pharmacy practice experience (ASHP-accredited residency may substitute month-for-month).

Responsibilities

• Collaborate with providers to individualize drug therapies.
• Perform comprehensive medication therapy review and monitor care.
• Monitor and modify drug regimens, including labs interpretation.
• Provide refill authorization with assessment of adherence, disease states, interactions, efficacy and safety.
• Act as a drug information resource for providers, nursing staff, and patients.
• Ensure access to specialty medications with insurer support.
• Educate patients on self-management of disease and therapy.
• Maintain drug therapy monitoring systems and QA programs.
• Assist in dispensing duties across clinic, infusion, and retail settings.
