Fred Hutchinson Cancer Center in Seattle seeks a Senior Pharmacist to join a multidisciplinary care team. You will provide pharmacotherapeutic recommendations, perform medication reviews, and support clinic patients with access to specialty medications.

The role blends direct patient care with research, education and practice management, involving collaboration with providers, nursing and research staff while working Monday–Friday at our South Lake Union location with remote options.

Formación

• Graduation from an accredited School of Pharmacy.
• State of Washington Pharmacist License.
• Two years of experience in pharmacy practice (ASHP residency may substitute).
• PharmD degree preferred.

Responsabilidades

• Collaborate with providers to individualize drug therapies.
• Conduct comprehensive medication therapy reviews.
• Monitor and adjust drug regimens with laboratory testing.
• Provide refill authorization with adherence assessment.
• Serve as drug information resource to staff and patients.
• Ensure access to specialty medications with insurers when needed.
• Identify under-treated patients for optimization.
