This full-time day-shift role focuses on complex cases, discharge planning, and care coordination across teams, with on-call duties including holidays and weekends.

Requirements include an RN license in Virginia or through a multi-state compact, a BSN or higher, BLS, and 3 years of nursing with 3 years of case management.

Qualifications

• RN-Bachelor's Degree or higher required.
• BLS certification required.
• Case Management certification (ACM/CCM/CCCTM/CMAC/CMGT-BC) within 1 year of eligibility.
• Virginia license or Multi-State Compact license.
• Minimum 3 years of nursing experience, 3 years in case management preferred.
• Hospital/acute care setting experience.

Responsibilities

• Lead daily inpatient case management operations and coordinate work quality.
• Oversee discharge planning, care coordination, and cross-team collaboration.
• Provide mentorship and feedback on hiring and evaluations.
• Maintain fiscal accountability for department activities.
• Participate in on-call duties including holidays and weekends.
