• Maintains strong clinical knowledge and serves as a resource to the healthcare team.
• Collaborates with community case managers to ensure safe continuum of care.
• Conducts assessments and interventions for patients at risk of readmission.
• Participates in daily interdisciplinary rounds to direct care progression.
• Pursues professional growth and attends educational programs.
• Attends departmental meetings and shares new topics and ideas.
• Provides education and orientation to new case management staff as needed.
