Job summary

Baystate Health is seeking a part-time RN Case Manager to coordinate clinical care, quality, and financial outcomes for a designated patient population. The role leads the interdisciplinary team in developing complex discharge plans and ensures continuity of care across community and post-acute settings.

The ideal candidate has 3–5 years of nursing experience, BSN preferred, and strong skills in utilization review, care coordination, and communication with payers and physicians.

Qualifications

• BSN preferred
• 3–5 years clinical nursing experience
• Hospital Case Management and care coordination experience strongly preferred

Responsibilities

• Facilitates patient care plans as called for in the plan of care.
• Leads daily patient care huddles to direct timely progress along the continuum.
• Manage inputs from health care professionals and conduct assessments to determine patient needs.
