Job summary

Baystate Noble Hospital is seeking an experienced Manager for Hospital Case Management to oversee daily operations, develop the case management team, and collaborate with physician leaders to coordinate high-quality care and efficient utilization management.

The role includes discharge planning, performance management, and advancing a contemporary case management model across the health system, with a focus on staff development and program growth.

Qualifications

• Minimum 5 years clinical nursing experience.
• Three years Case Management experience required.
• Previous supervisory and team development skills strongly preferred.

Responsibilities

• Oversee planning and daily operations of hospital case management.
• Partner with physician leaders and care teams to improve coordination of clinical care and utilization management.
• Lead discharge coordination, ensure quality care and favorable financial outcomes, and manage denials and appeals.
• Supervise staff performance, orientation, competency development, and annual reviews.
• Develop and implement programmatic competency requirements and staff education.
• Recruit and hire staff and build a diverse, high-performing team.
