Sentara Careplex Hospital in Hampton, VA is seeking an Inpatient Case Manager, RN for days shift (8 hours, 40 hours weekly). The role focuses on comprehensive care management and patient transitions to appropriate care levels, improving outcomes and LOS.

Inpatient Case Manager, Registered Nurse

Shift: Days/8 Hours

Weekly Scheduled Hours: 40

Overview

Responsible and accountable for the provision and facilitation of comprehensive care management, services, and quality outcomes for patients across the healthcare continuum, primary focus on the patient transition to the appropriate level of care while facilitating clinical appropriateness and Length of Stay. Directly support those in-hospital patients identified in need of comprehensive case management. Promotes effective utilization and monitoring of health services, collaborates and communicates with the healthcare team and patient/caregiver to manage care and transitions. Develops and/or implements a comprehensive care plan based on assessment and evaluation to include the physical, mental, and psychosocial needs of the inpatient/caregiver. Functions in one of the following practice settings: Hospitals Only.

Scope: The Emergency Department, Registered Nurse partners with ED providers, nursing, inpatient case management, ambulatory care management, and health plan teams to identify members who can be safely supported through observation, outpatient follow-up, home-based services, community resources, or care management programs.

Education

• RN-Associate's Degree OR

Certification/Licensure

• Registered Nurse (RN) License - Compact/Multi-State License OR
• Registered Nurse (RN) License - Single State – Virginia
• Basic Life Support (BLS) - Certification - American Heart
• Case Management (ACM, CCM, CCCTM, CMAC, or CMGT-BC) certification is required within one year of eligibility.
• Association (AHA) BLS within 90 days of hire.

Experience

• 1 year of nursing experience (required)
• 1 year acute care experience (preferred)
• 1 year case management experience (preferred)

We provide market-competitive compensation packages, inclusive of base pay, incentives, and benefits. The base pay range for full-time employment is $89,544.00 - $136,468.80 annually. Additional compensation may be available for this role, such as shift differentials, on-call pay, shift premiums, extra-shift incentives, or bonus opportunities. Compensation within the range may vary based on qualifications, experience, location, market conditions, and business needs.
