Sentara CarePlex Hospital and Sentara Louise Obici Hospital in Hampton, VA are seeking an experienced Inpatient Case Management Supervisor, RN to lead inpatient case management teams across both campuses. This full-time day-shift role focuses on high-quality discharge planning, patient advocacy, and staff development in a fast-paced acute care environment.

The position requires a BSN, active RN license (VA or compact), BLS within 90 days, and 3+ years in case management with supervisory

Qualificações

• BSN required; current RN license (VA or Compact/Multi-State)
• BLS certification from American Heart Association within 90 days of hire
• Specialty certification within one year (ACM/CCM/CCCTM/CMAC/CMGT-BC)
• At least 3 years of case management experience
• Supervisory experience preferred

Responsabilidades

• Supervise day-to-day workflow and performance of the case management team at both hospital locations
• Ensure effective discharge planning and seamless inpatient care coordination
• Support length-of-stay management and patient flow improvement initiatives
• Coach, mentor, and provide constructive feedback to staff
• Partner with leadership/HR to address employee relations and maintain policy compliance
• Maintain compliance with departmental standards and hospital policies
