Job summary

The role requires a Texas RN license (or LMSW in Texas; LCSW preferred), CCM/ACM/FAACM certification, and at least 5 years in utilization/case management plus 5 years of supervisory experience. Relocation assistance is available.

Qualifications

• Current and valid license: Registered Nurse in Texas (or compact state) or Masters Social Worker (LMSW) in Texas. LCSW preferred.
• Certification: CCM, ACM, or FAACM required.
• Experience: Min five (5) years in utilization management, case management, discharge planning, or related cost/quality management. Three (3) years hospital-based nursing or social work preferred.
• Supervisory experience: At least 5 years of supervisory leadership.

Responsibilities

• Drive the implementation and execution of the inpatient case management program, overseeing Care Facilitation, Utilization Management, Case Management, and Discharge Planning.
• Ensure compliance with industry standards, regulations, and best practices, optimizing patient care quality and cost-effectiveness.
• Collaborate closely with interdisciplinary teams to streamline processes, enhance patient outcomes, and facilitate seamless transitions of care.
