Qualifications

• Strong communication, prioritization, and knowledge of CMS guidelines and utilization management.

Responsibilities

• Perform comprehensive patient assessments and develop discharge plans with patient-family involvement.
• Analyze and report data on patient outcomes and resource use to committees.
• Coordinate care transitions and discharge planning for clinically complex patients.
• Provide clinical guidance and mentorship to care management team to improve care quality.
• Advocate for patients to ensure needs are met while optimizing resources.
• Lead multidisciplinary rounds and collaborate with social work to remove discharge barriers.
• Engage with physician groups to discuss best practices and ensure regulatory compliance.
