Qualifications

• Bachelor's Degree in Nursing or Master’s Degree in Social Work required.
• Active RN or SW licensure in applicable state.
• ACM/CCM or RN-BC/C-SWHC certification within 1 year, if applicable.

Responsibilities

• Oversee daily operations of care management and social work programs at assigned sites.
• Monitor staff schedules, PTO, and time cards to ensure continuity of services.
