Job summary

• Ensure integrated, coordinated active treatment programs across providers.
• Maintain confidentiality and protect client rights.
• Schedule ISP meetings and prepare documentation for approvals.

Responsibilities

• Coordinate ISP meetings and present ISPs for COS approvals.
• Develop and monitor Implementation Plans and data tracking sheets.
• Oversee staffing coordination and on-call responsibilities.
• Participate in Human Rights Committee and accreditation activities.
