Job summary

The Economic Opportunity Council of Suffolk Inc in Patchogue, NY is seeking a HUD Rapid Re-Housing Senior Case Coordinator to deliver case management and housing support to participants experiencing homelessness, helping them obtain and maintain permanent housing.

This role also mentors Case Managers, coordinates services, and works with partners to resolve barriers to service delivery, while ensuring compliance with program requirements.

Qualifications

• Two years of case management experience.
• Bachelor’s degree or equivalent.
• Experience with homelessness population a plus; knowledge of mainstream benefits.
• Ability to develop community resources.

Responsibilities

• Provide case management and housing support to participants experiencing homelessness to obtain and maintain permanent housing.
• Mentor and supervise Case Managers to resolve complex cases and improve service delivery.
• Coordinate services, advocate for benefits, and arrange housing searches and landlord outreach.
• Maintain confidential files and HMIS entries, and monitor progress toward housing stability.
