Options for Community Living, Inc. is hiring a Housing Specialist to assist in rapid rehousing, coordinating support services for clients, and helping homeless individuals and families obtain and maintain housing. This role requires field visits, landlord liaison, and collaboration with case managers.

The position is based in Uniondale with possible travel to Ronkonkoma offices; it involves flexible scheduling and non-exempt pay. Experience in RRH and knowledge of housing resources are valued.

Qualifications

• Associate's degree in health or human services or RN/LPN certification with 2 years of qualifying experience.
• Bachelor's degree in health or human services with 2 years of qualifying experience.
• Experience with families preferred.
• Valid driver's license, safe driving record, valid auto insurance and access to a vehicle.
• Verifiable full or part-time case management or case work with homeless, HIV/AIDS, mental illness, or chemical dependence.

Responsibilities

• Assist Program Supervisor in coordinating support services to clients in RRH program.
• Help homeless individuals and families obtain and maintain housing.
• Work with clients to reduce barriers and prepare for independent living.
• Assist with landlord meetings and housing searches.
• Provide outreach to community resources and job readiness programs.
• Engage landlords, maintain realtor/vacancy lists, track referrals and rent contributions.
• Provide housing retention services including intake, assessment, and reassessment.
• Ensure access to housing support services, crisis intervention, and vocational readiness education.
• Make regular site visits to ensure health and safety and coordinate with providers.
• Maintain contact with referral sources, complete intakes, and manage waiting lists.
• Arrange DSS housing requests, inspect sites, and sustain relationships with DSS staff.
• Participate in case conferences at least quarterly.
• Assist in coordinating discharges and ensuring appropriate alternatives.
• Track client rent contributions and referrals for program matching.
• Connect clients with resources to increase job skills and readiness.
• Ensure clients obtain eligible benefits and coordinate with internal/external staff.
