Job summary

Acacia Network in New York City seeks a Case Manager Supervisor to oversee shelter-based social services, supervise Intake Services and Case Management teams, and collaborate with the Program Director to ensure DHS compliance and measurable outcomes.

You will drive client-centered care, coordinate medical referrals, monitor care plans, and support staff development through ongoing supervision and mentorship.

Qualifications

• Minimum five years’ experience with homeless/mixed income populations.
• Specialty in substance abuse and mental health issues.
• Knowledge of crisis prevention, intervention and resolution techniques.

Responsibilities

• Mentor Case Managers and promote professional development.
• Maintain updated case files with necessary documentation.
• Conduct and document one-on-one supervision of direct reporting staff.
• Ensure clients have appropriate medical insurance and referrals to medical providers.
• Oversee housing placements and care plans for clients.
• Submit accurate monthly DHS and ACACIA reports to the Program Director.
• Plan and organize program activities to meet contract and performance goals.
• Coordinate with other Case Manager Supervisors on shared practices and client progress.
• Support crisis prevention strategies and ensure a safe environment.
