Acacia Network in Queens, NY seeks a Senior Case Manager to supervise the Intake Services and Case Management team, ensuring effective social services delivery in the shelter and alignment with DHS requirements.

The role includes developing and tracking measurable outcomes, coordinating housing, substance abuse treatment, and mental health services, and working with the Program Director to maintain high-quality client care.

Qualifications

• Minimum of five (5) years’ experience working with homeless and/or mixed income population.
• Specialty in substance abuse and mental health issues.
• Knowledge of crisis prevention, intervention, and resolution techniques; match such techniques to circumstance and individuals.

Responsibilities

• Provide supervision to Intake Services and Case Management team to develop, implement, track and evaluate outcomes.
• Ensure clients have appropriate medical insurance and are referred to medical providers.
• Oversee timely assessment of clients at admission and update care plans; monitor housing and medical compliance.
