THRIVE seeks a dedicated professional to provide health and safety support to residents in an apartment program. You will help ensure access to community services, monitor behaviors, and document progress toward treatment goals.

The role may involve transporting clients using a personal vehicle when agency vehicles are unavailable. Responsibilities include coordinating service plans, assisting with daily living needs, and delivering restorative services as outlined in plans.

Formación

• Associate’s degree in the Human Services field is required, with two years related experience and/or training.
• Preferred: Bachelor’s degree in Human Services and two years related experience in mental health, QMHS.

Responsabilidades

• Report maintenance concerns to the Apartment Program Director immediately.
• Assist residents with basic needs: food, clothing, shelter, and medication adherence per physician’s instructions.
• Instruct residents on proper storage of medications and valuables.
• Record illnesses, accidents and repairs and communicate with supervisor.
• Monitor food, supplies and upkeep; assist residents with purchases and cleaning as needed.
• Develop and monitor service plans using Psychiatric Rehabilitation principles.
