Father Joe’s Villages is seeking a Supervisor of Case Management to lead a team serving residents in Permanent Supportive Housing. The role focuses on crisis response, staff development, and strengthening service quality to promote housing stability.

The ideal candidate has a Bachelor’s degree in a relevant field, 5+ years in case management, and at least 1 year of supervisory experience. Strong collaboration with providers and a trauma-informed, client-centered mindset are essential.

Qualifications

• Bachelor’s degree in Social Work, Human Services, Psychology, or related field, or equivalent experience.
• 5+ years of experience in case management or related human services setting.
• 1+ year of lead or supervisory experience supporting employee development and service delivery.
• Experience serving individuals or families experiencing homelessness or housing instability.

Responsabilités

• Partner with staff to address complex client needs, respond to crises, resolve conflicts, and identify practical next steps for housing stability.
• Facilitate Multidisciplinary Approach to Services (MAS) meetings and coordinate with program staff, property management, and partners.
• Review caseloads, assessments, service plans, referrals, client contacts, and documentation for accuracy and compliance.
• Monitor program data and housing outcomes to identify trends and improve services.
• Guide staff in developing service plans and connecting clients with resources using Housing First, trauma-informed, and strengths-based approaches.
• Support daily operations including personnel documentation, scheduling, training, and other administrative responsibilities.
