Job summary

Father Joe’s Villages is seeking a Case Manager I for the RISE Sober Shelter in San Diego to help clients access housing, health, and supportive services. You will conduct biopsychosocial assessments, develop individual plans, and coordinate with partners to meet immediate needs while building toward stability.

You will work across office, program, and community settings, maintaining records and participating in team meetings with focus on compassionate, client-centered care.

Qualifications

• Bachelor's degree in social work, psychology, sociology, or related field (or equivalent).
• 1 year of experience with underserved populations preferred.
• Understanding of homeless interventions and social services.

Responsibilities

• Conduct client intakes, orientations, and biopsychosocial assessments to identify needs, strengths, and service goals.
• Develop and implement individualized case plans supporting housing stability, income, health, and self-sufficiency.
• Connect clients with housing resources, public benefits, employment services, medical and mental health care, and recovery resources.

Help create a path forward.At Father Joe’s Villages, helping people move out of homelessness starts with meeting immediate needs, building trust, and creating connections to the resources that support lasting stability. As a Case Manager I in the RISE Sober Shelter, you will work directly with individuals experiencing homelessness. The Case Manager I helps clients navigate community resources, access essential services, and take steps toward housing, health, and self-sufficiency. Through motivational interviewing, ongoing case management, and collaboration with community partners, you’ll help clients identify work toward their personal goals.The RoleConduct client intakes, orientations, and biopsychosocial assessments to identify needs, strengths, and service goals.Develop and implement individualized case plans that support housing stability, income, health, and progress toward self-sufficiency.Connect clients with housing resources, public benefits, employment services, medical care, mental health services, substance use recovery resources, and other community support.Respond to client needs and crisis situations, seeking guidance from supervisors, and escalating complex concerns when appropriate.Collaborate with team members and community partners to coordinate services and support client goals.Maintain accurate, timely client records, assessments, service plans, and program documentation for data collection and reporting purposes.Participate in team meetings, case reviews, required training, and other activities that support service delivery.What You BringBachelor's degree in social work, Psychology, Sociology, or a related field; an equivalent combination of education and work experience may be considered.One (1) year of experience working with underserved populations in social service or a related setting preferred.An understanding of the principles and practices of homeless interventions and social services.Knowledge of applicable laws related to housing intervention and social services.Valid California driver’s license.Bilingual in Spanish and English preferred.Why Father Joe’s VillagesYou will be a part of a mission-driven team working to prevent and end homelessness through compassionate and comprehensive services.
