Job summary

Apreva Hospice in San Diego, CA seeks a dedicated RN Clinical Supervisor to oversee hospice staff and coordinate patient care from our San Diego location. You will guide clinical decisions, lead team conferences, and ensure high-quality end-of-life care with strong communication and empathy.

The ideal candidate holds an active CA RN license (BSN preferred) and has prior hospice or home care supervisory experience, demonstrating leadership and clinical excellence.

Qualifications

• RN licensure in the state of operation (CA) required.
• Previous hospice or home care supervision experience preferred.
• Strong leadership, communication, and clinical decision-making skills.

Responsibilities

• Oversee hospice staff and coordinate patient care plans.
• Review referrals, assign staff, and guide clinical decisions.
• Lead team case conferences and ensure holistic care.
• Participate in hiring, onboarding, and training of clinicians.
• Maintain thorough clinical documentation and quality improvement.
