Qualifications

• Registered Nurse with current, unencumbered licensure in the state of operation.
• BSN preferred for advanced clinical leadership.
• Minimum hospice or home care experience required.
• Minimum 2 years of supervisory or management experience preferred.

Responsibilities

• Review new patient referrals and assign hospice staff.
• Guide clinical staff during home visits and case reviews.
• Develop patient care plans and set treatment goals.
• Lead team conferences to ensure coordinated care.
• Perform chart audits and support quality improvement.
• Participate in hiring, onboarding and training of staff.
• Support staff development and continuing education.
• Assist in policy development and uphold standards.
• Engage in community outreach and promote services.
