Job summary

Apreva Hospice in San Diego, CA is seeking a dedicated Hospice RN to join our team. You will provide patient-centered, 1:1 care in a supportive environment, assess complex needs, and develop individualized care plans.

As an RN Case Manager you will deliver direct nursing care, manage medications and symptoms, and educate families. The role offers stability, team collaboration, and growth opportunities with a reputable hospice provider in the San Diego area.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with an insured automobile as required.
• Possess and maintain current CPR certification in states where required.

Responsibilities

• Assess/monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with the patients' wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.

USD $55.00 - $60.00 / Hour
