Qualifications

• Must hold a current RN license to practice in the state.
• BSN preferred or minimum 1 year of professional nursing experience in the last 3 years.
• Previous hospice/home care experience and 2+ years in management or supervisory roles.
• Complies with professional standards and practice.
• HCHB and PointCare experience preferred.
• Excellent nursing judgment and communication skills.
• Demonstrates knowledge of pain/symptom management.
• Valid driver’s license and insured vehicle.
• CPR certification maintained.
• Strong observation and written communication skills.

Responsibilities

• Receives case referrals and assigns hospice personnel to cases.
• Reviews cases via home visits, conferences, records, and clinician services.
• Reviews medical diagnoses, prognosis, medications, and course.
• Manages medical records and workflow in EMR system.
• Attends case conference meetings to coordinate care.
• Conducts quarterly record reviews and shares findings with leadership.
• Participates in community activities to promote hospice services.
• Participates in quality assessment and performance improvement teams.
• Performs other duties as delegated by Clinical Director.
