Job summary

Affinity Hospice Holdings, LLC is seeking a Clinical Manager to oversee and coordinate hospice clinical services. You will direct patient care, supervise an interdisciplinary team, and partner with the Medical Director to meet service standards.

The role requires RN licensure, a BSN preferred, and at least two years of management experience in hospice or home care. Full-time position with comprehensive benefits and ongoing staff development opportunities.

Qualifications

• RN licensed in the state of employment.
• BSN preferred; NLN-accredited program.
• Minimum 2 years of management or supervisory hospice/home care experience.
• Current CPR certification.

Responsibilities

• Review referrals and assess patient needs, including home visits.
• Assign hospice staff and coordinate with Medical Director on eligibility.
• Oversee patient cases via home visits, conferences, and records review.
• Provide guidance to clinicians to improve care delivery.
• Lead case conferences to ensure coordination across disciplines.
• Support hiring, onboarding, and staff mentorship.
• Contribute to staff education and ongoing training.
• Ensure compliance with professional standards and regulations.
